Complex vessel anatomy or procedures when different wire shapes are needed add time and challenges to aneurysm procedures with current available 0.014'' microwires. The SmartGUIDE (Artiria Medical, Geneva Switzerland) was developed to overcome these challenges by having a novel deflectable-tip design with 3 different deflection positions. This single center study reports the initial experience with the SmartGuide.
Different aneurysm treatment procedures using the 0.014'' SmartGUIDE were included for analysis. Prospectively collected data including clinical outcome, technical success, procedure time, and periprocedural complications (thromboembolic, hemorrhagic, vessel dissection or perforation, infection) were analyzed.
Overall, 30 patients were included and SmartGUIDE deflectable-tip guidewires were used in all 30 aneurysm procedures including flow diverter (Pipeline shield, pipeline vantage, Surpass Elite, FRED X), coiling, stent-coiling and intrasaccular treatments (WEB, Contour, Seal it). No compatibility issues were found using a variety of 0.0165’- 0.027’ microcatheters and the wire reached the target in all cases. No device-related complications were recorded intraoperatively or at patient discharge. In 6 of the cases, the wire was used not only to deliver the intended treatment to the target location but also for postprocessing of deployed stents by deflecting the tip to position III.
This initial single-center experience shows the feasibility and safety of the SmartGUIDE deflectable-tip microwire in various neurointerventional aneurysm procedures to reach the target lesion and to assist with post-deployment processing of flow diverters
